On federal supervised release but need liver transplant and medical care that prevents me from completing conditions of supervised release.

A Judge is the only person who can alter the condition of your release. Get supporting documentation of your medical condition and the reason why you cannot meet the conditions of your release. Have your attorney approach the Judge and request the modification.
